Athlete Paperwork

Student athletes in all sports must have a physical examination along with all forms requested in the Vivature portal prior to any athletic participation.  Athletic participation includes any official team practices, voluntary workouts with coaches or other athletes, any team structured activities, and any use of the university’s weight room and fitness center. The due date for fall student-athletes is July 26th, 2024 and for semester transfer student-athletes is January 25th, 2025. In the event that you do not complete ALL forms by the specified date, there will be a $25.00 late processing fee on your student account. Please call if you have any questions.

If you have questions or troubles with the Vivature portal, please contact Tama Crane at tpcrane@vivature.com. Please read the instructions thoroughly. All other questions should be directed to your sport’s athletic trainer.

Vivature Portal Forms-

  • FORM A: Medical History
  • FORM B: Consent form
  • FORM C: Verification of Current Medical Coverage
    • This requests a legible copy of your health care insurance card be electronically uploaded to the NExTT PIC app via your iPhone or other mobile device and manually uploaded to the Vivature portal. Follow the instructions online. Please secure a card for your personal use! Providers require this upon admittance for services.
  • FORM D: Concussion Waiver
    • This states that you have read and understand the “concussion fact sheet” link located on the Baker Athletics Website.
  • FORM E: HIPAA FORM
  • FORM F: Secondary Insurance Agreement
  • PHYSICAL EXAMINATION (Link to Physical Here) **MUST USE THIS FORM**
    • Physical Examination is completed yearly and must be administered and signed by one of the following: Doctor of Medicine (MD), Doctor of Osteopathy (DO), Physician Assistant (PA), or Nurse Practitioner (APRN). Upon completion- fax or mail to address below. Any physician ordered tests must be completed with documentation returned to sports medicine department prior to athletic participation. Physicals are valid for one year from the date of exam.
    • Your physical examination must be administered by a provider listed above. Other health providers will not be accepted and a subsequent examination will be required prior to any participation.
    • Sports medicine will be offering physicals at Baker University in Mabee Gym on Friday, Aug. 2nd from 4-6pm. These physicals are $20 cash or check only and must be paid on arrival. You must sign up for these timeslots prior to July 26th as well as complete all online forms before arrival. Comfortable clothes you can move in must be worn. See sign up times below:
  • Primary Insurance is required for every student athlete. All student athletes are required to notify Baker University Sports Medicine Staff immediately should their insurance policy change or become invalid/inactive. All athletes must have proof of primary health/medical insurance effective August 1, 2024 or prior to the start of the athletic season. This full coverage insurance must be carried throughout the schedule of athletic practices, contests, and participation, in and out of season. Any insurance lapse will result in my removal from participation, team roster, and the loss of continued athletic scholarship assistance.
